Patient update! 3/22/2017

Hello!

We wanted to take a moment to thank you all so much for bearing with us throughout this transition, and provide you with some updated information.

Online scheduling

We now have appointments available for existing patients online through our Updox portal! We will always offer a few same-day appointment slots for those who need routine refills and require no medication changes. Video appointments are also now available via Skype.

You can sign up and start scheduling below and click “register”. If you encounter any difficulty, please e-mail us at info@villagefamilyclinic.com.

Follow-up/ new patient visits:

Our pre-appointment paperwork is now available online!

  • If you are coming into the office for an appointment, please fill out the “follow-up” form.
  • If you have scheduled a Skype appointment, please fill out the “virtual visit” form.
  • If you are a new patient, please fill out the new patient forms #1, #2, and #3 on our patient portal.

Medication refills:

We are currently unable to provide electronic prescriptions for controlled substances.

If you have had an appointment within the past 3 months and require a refill on your medication, please either ask your pharmacy to fax us a request at (425) 274-7294, or send us a portal message/e-mail with the name/dosage of the medication you need refilled.

Prescriptions may be either picked up in the office during business hours, or mailed to your home. Please allow 48 hours for all refills.

Payment:

We offer a 20% discount to self-pay patients that pay their bill at the time-of-service.

We are currently able to accept payment by check, cash, or our Paypal. If you wish to use a HSA card, we will alert you shortly once our new billing proram is set up.
