eXISTING PATIENTS
Schedule your appointment by reading instructions below! Once stable on your medication, you may schedule a Doxy visit every other time that you need a refill.
Self-scheduling:
Please make sure you do not have a balance on your account from prior visits before scheduling yourself for an appointment.
Visit our patient portal to view available appointment times. If you do not see an option the portal that works for you, send us an e-mail with your general availability. There are additional times open that we do not post online. If you are unable to access the patient portal, send us an e-mail at info@villagefamilyclinic.com. We will be in touch with you within 24-48 hours to confirm your appointment.

SCHEDULE A DOXY VISIT!
Virtual visits visits are only for patients ages 18 and older. If you are stable on your medication, you may have a virtual visit via Doxy every other appointment. (Every 6 months) Virtual visits are not for medication changes. Please schedule an in-person office visit if you are interested in changing medications/dosages.

#2: Patients with insurance: Please your co-pay before your visit here. If you are a self-pay patient, the cost of a virtual visit is $96 when paid before your visit. If you choose to pay within 30 days, the charge is $120.
